This hospice app provides an easy way for healthcare providers and others to refer patients to Alive Hospice in the Middle Tennessee area. The app also provides information about hospice eligibility and diagnoses. Referrals submitted through this app will be received and processed by the Intake department at Alive Hospice. All patient information processed through this referral app will be managed with HIPAA compliance.
